Türkiye Cumhuriyeti sağlık hizmetleri finansman yapısı ve evrimi, ekonomik ve siyasi değişimlerle yakından ilişkilidir. Sağlık hizmetlerinde Cumhuriyet öncesi dönemden başlayarak değişen iktisadi ve siyasi tablo, savaşlar, salgınlar ve kapitalizmin geçirdiği aşamalar belirleyici olmuştur. Kamu sağlık sigortacılığı, iktisadi gelişmeler ve ihtiyaçlarla gelişmiş, 1960’lı yıllarda sağlık hizmetlerinin sosyalizasyonunda önemli finansal kaynak oluşturmuştur. 1980’li yıllarda kapitalizmin neoliberal dönüşümü ve sermayenin küreselleşmesi, sağlık hizmetlerini sosyal bir politika olmaktan çıkarmıştır. Bu süreç 1982 Anayasası ile resmileşmiş, 21 yıllık AKP döneminde uygulanan Sağlıkta Dönüşüm Programı ile süreç tamamlanmıştır. Sağlık hizmetlerinde devletin rolünün azalması ve özel sektörün desteklenmesi, sağlık hizmetlerinin piyasalaşmasına ve ticarileşmesine yol açmıştır. Sağlıkta metalaşma olarak tanımlanabilecek bu süreç, sağlık hizmetlerinin finansmanında özel sağlık sigortacılığının büyüdüğü, cepten ödemenin arttığı bir dönemi getirmiştir.

The financing structure and evolution of healthcare services in the Republic of Turkey are closely linked to economic and political changes.The economic and political landscape changing from the pre-Republic period onwards, wars, epidemics, and phases of capitalism have been decisive in the healthcare services. Public health insurance has developed with economic advancements and needs, becoming a significant financial resource for the socialization of healthcare services in the 1960s. The neoliberal transformation of capitalism and globalization of capital in the 1980s removed healthcare services from being a social policy. This process was formalized with the 1982 Constitution and completed with the Health Transformation Program implemented during the 21-year AKP period. The reduction of state’s role in healthcare services and the support for the private sector has led to the marketization and commercialization of healthcare services. This process, which can be defined as the commodification of healthcare, has ushered in an era where private health insurance has grown, and out-of-pocket payments have increased.
